No one will ever know what you told us because:
- we are not part of the program you attended,
- your responses will be combined with responses from other people taking part in the study and no names are ever revealed, and
- we have a Federal Certificate of Confidentiality which means we can’t be forced to give out any of your information, even under a court subpoena.

Between 6 and 12 months ago you started this study. A staff person from the University of Kentucky will call you to do the second part of the study with a quick survey. The survey will ask about:
what you liked and didn’t like about the program,
how you think the program could be better, and
how you are doing now.
